Health Room News 2025-2026
Welcome to the 2025-2026 school year and to the Rosewood Health Room.
The health room's primary goal is to provide quality care for those who may be feeling unwell, have experienced a minor injury, or require medical attention during the school day. The health room always wants to promote a healthy, respectful, and nurturing atmosphere where students can receive the care they need and return to class feeling their best.
I am Diane Reaves, and I serve as your child's School Nurse. I have worked at Rosewood Elementary School for 28 years. I attended Rosewood Elementary in grades 1-5. My children both attended Rosewood Elementary for their elementary education. I served as a parent volunteer and homeroom mom while they attended Rosewood and became full-time nurse when my son was in 3rd grade. Rosewood is a very special place because of the people we serve and the staff I work with. I look forward to partnering with you to ensure your student has a safe and healthy year!
Things that you can do to help your student to be safe and healthy
*Please remember to update & keep your contact information current. If your contact information changes during the school year, please notify us. When a student becomes ill or injured, the school will notify you or the alternate contact person to pick up your student. It is especially important that we have the name and phone number of someone (who has agreed) that we can reach during school hours if an emergency occurs. All contact numbers MUST be kept current. If your child has any changes in health status (injuries and/or surgeries included) during the school year, please notify the nurse.
Important Reminders regarding medication at school
If possible, the parent should give medications before or after school hours.
The Health Room does not stock any medications; therefore, you must bring any medication that is necessary for your child.
- Initial doses of a medication that a child has never taken before should NOT be given at school.
- All medications require a permission form to be fully completed PRIOR to any medication given during the school day. Medication forms are on the Rosewood Website under the Nurse tab(below) or outside of the Health Room door.
Over the counter medications such as Tylenol/Advil require the signature of the parent/guardian. These medications MUST be in the original container and the permission form completed to match the label instructions and age requirements.
Prescription medications require the signature of the healthcare provider AND the parent/guardian BEFORE it can be given at school. Prescribed medications MUST be in the properly labeled container from the pharmacy, including the name of the student, name of the drug and time to be given. The instructions on the bottle must match the medication permission form.
- A separate form must be completed for each medication.
- Prescriptions and medication permission forms must be completed each school year.
- Schools may decline to administer certain medications if deemed inappropriate for a school setting.
- Medications that make students drowsy and unable to participate in educational activities may not be appropriate for school administration.
- A responsible adult should deliver the medicine and permission form to the school. The medication must be in the original container with the manufacturer's label intact.
- Students cannot transport medication home at the end of the day. If the student needs the medication after school hours, a responsible adult must pick up the medication.
- Please do not send any medication in your child's pocket, lunch box or wrapped in foil, saran wrap or plastic bags to be given/taken.
- Parent/Guardian must count medication with the school nurse when bringing in controlled substance medications.
The Health Room is a steppingstone to getting your student back to class or home when they are sick or injured. It is not for students requiring extended care or for students who want to sleep because they did not get enough sleep during the night.
Student concerns are triaged upon arrival to the health room with the goal of the student returning to class as quickly as possible to continue his/her educational activities.

Vision and Hearing Screening form - Copy.docx Medical Form For Student Requiring Special Meals Due to Food Allergy or Intolerance (PDF) Permission for School Administration of Prescription Medication (Below) Prescription medications are medicines that require a written prescription from a health care practitioner. In order for a child to be given a prescription medicine at school, the child's health care practitioner and the child's parent or guardian should sign a permission form. The parent or guardian signature will be provided on the same day the health are practitioner signs or signed after the date of the health care practitioner signature.- A permission form for the prescription medicines is provided below. Because the instructions for some medications require more space than is provided on the form, some health care practitioners may prefer to use a practice specific form.
- Schools may have special rules for prescription medicines and may require parents or guardians to sign additional forms.
- A responsible adult should deliver the medicine and the permission form to the school. The medicine must be in the original container with the label on it.

Permission for School Administration of Non-Prescription Medication (Below) Non-Prescription medications are medicines that you can buy without a written prescription from a health care practitioner. Non-prescription medications are sometimes called "over-the-counter" medicines.- In order for a child to be given non-prescription medicines at school, the child's parent/guardian must sign a permission form. A permission form for non-prescription medicine is provided below. Schools may have special rules for non-prescription medicines and may require parents or guardians to sign additional forms.
